Value-based Primary Care Physician role to manage chronic conditions of high-risk geriatric patients Monday-Friday (no weekends or evenings). Spend 30-45 minutes with your patients to manage all chronic conditions alongside a supportive team of providers and office operations staff. Responsibilities:

The Physician delivers primary health care and patient care management in a residential setting.

Qualifications:

Experience in Geriatrics or complex patients preferred Active/unrestricted medical license and DEA Board Certified or Board Eligible EMR experience Must maintain a valid driver's license and good driving record
