You'll provide on-site primary care to seniors living in assisted living, memory care, independent living, and skilled nursing communities throughout Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities. Instead of seeing a revolving door of patients, you'll have the opportunity to build a consistent patient population and follow their health over time . You'll know their histories, recognize changes, develop relationships with families and community nurses, and become a provider people know and trust. More Continuity. More Autonomy. More Meaningful Medicine. This isn't a traditional clinic position. You go to your patients , providing primary care directly within the communities where they live. You'll manage medically complex older adults with conditions such as diabetes, CHF, COPD, hypertension, dementia, and other chronic illnesses , while also addressing acute concerns, preventative care needs, and transitions following hospitalization. Because you're following patients over time, you have the opportunity to understand the bigger picture. You'll know their baseline, communicate directly with the nurses and caregivers who see them every day, and recognize when a subtle change may require intervention. You'll have the autonomy to assess, diagnose, treat, manage chronic conditions, and develop individualized care plans , while having the resources and collaboration of a physician-led medical group behind you. Mobility limitations, chronic illnesses, cognitive changes, transportation challenges, and the complexity of coordinating appointments can all become barriers to consistent medical care. Instead of asking patients to continually navigate those barriers, we bring primary care directly to them . You'll travel throughout Owatonna, Rochester, Albert Lea, and surrounding communities, providing care within assisted living, memory care, independent living, and skilled nursing settings . You'll work closely with community nurses, caregivers, families, therapists, pharmacists, and other members of the care team to understand what's happening beyond a single appointment. That allows you to practice proactive medicine , identifying concerns earlier, managing chronic conditions more consistently, and helping prevent avoidable emergency department visits and hospitalizations. What You'll Be Doing Provide comprehensive on-site primary care throughout senior living and long-term care communities Travel throughout Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities Manage your own panel of older adult and medically complex patients Diagnose and manage acute and chronic medical conditions Perform comprehensive assessments, annual wellness visits, preventative care, and routine follow-ups Develop individualized treatment plans based on patient needs and goals Manage medications and chronic disease treatment plans Recognize changes in condition and intervene appropriately Coordinate care following hospitalizations, rehabilitation stays, and specialist visits Collaborate with physicians, nurses, therapists, pharmacists, caregivers, and senior living teams Educate patients and families regarding diagnoses, medications, and treatment plans Help identify concerns early and reduce avoidable hospitalizations and readmissions Maintain accurate and timely clinical documentation Clinical Backgrounds That Transition Well We welcome Physician Assistants with experience in Primary Care, Family Medicine, Internal Medicine, Urgent Care, Hospital Medicine, Geriatrics, Long-Term Care, Skilled Nursing, Home Health, Community-Based Medicine , and other settings caring for medically complex adults. You don't need to come directly from senior living. If you have strong clinical judgment, enjoy caring for older adults, and want the opportunity to build longer-term patient relationships , we want to hear from you. What We're Looking For Required Qualifications Graduate of an accredited Physician Assistant program Active Minnesota Physician Assistant (PA-C) license in good standing Strong assessment, diagnostic, and clinical decision-making skills Confidence managing patients with multiple chronic conditions Ability to practice autonomously while collaborating within a physician-led team Strong communication and care coordination skills Passion for caring for older adults Reliable transportation and willingness to travel throughout Southern Minnesota Preferred Qualifications Experience managing medically complex older adults Primary care or chronic disease management experience Geriatric, long-term care, or community-based medicine experience Assisted living, memory care, or skilled nursing experience Advanced wound care experience Wound Care Certification (WCC) is a plus Build the Kind of Practice Patients Remember This role gives you something that can be difficult to find in traditional healthcare: autonomy without isolation, productivity without an assembly-line clinic, and the opportunity to develop genuine continuity with your patients .
