Job Summary We are seeking a dedicated and compassionate Qualified Intellectual Disabilities Professional (QIDP) for Community Day Services to empower individuals with developmental and intellectual disabilities to lead fulfilling, independent lives. Non-Exempt Function:
Responsible for all aspects of the day-to-day programming of the Assigned Home and Community Based Waiver program. Work Hours Monday-Friday 8-4PM Reports to:
HBS Director Supervises:
DPSs as assigned
Qualifications:
  • REQUIRED DHS QIDP
    approval (degree
  • and 1 year experience)(
  • bachelor's degree in a human services field, including, but not limited tosociology, special education, rehabilitation counseling or psychology)
  • Strong working knowledge of related rules & regulations (HBS, State & CARF)
  • Able to certify in First Aid/CPR/AED
  • Exceptional documentation skills
  • Exceptional verbal and written communication skills
  • Working knowledge of Microsoft Word, Excel and electronic communication
  • Exceptional time management skills
  • Able to work independently for long periods of time
  • Previous staff supervisory experience reqiured
  • Self starter
  • Able to coordinate and work with a team.
EssentialJob Duties:
1) Ensure client health, safety, and wellness1) Assure all needs, desires and choices are addressed. 2) Provide immediate and appropriate crisis management. 3) Fulfill all funder requirements(HBS and State). 4) Fulfill all CARF requirements. 5) Ensure an exceptional level of confidentiality. 6) Responsible for annual
Implementation Strategy:
Assist creation of yearly outcomes Attend yearly outcome meeting when face-to-face time requested. Ensure all appropriate paperwork relating to outcomes is completed Ensure rights during the outcomes process are not being violated Communicate with ISC all people desired by the individual and necessary to the planning process are included in the PCP process/meeting, if applicable. 7) Ensure appropriate adaptations are in place to fit each individual's needs. 8) Assure Implementation Strategy is reviewed annually. Annual ICAPs completed, if requested by ISC. 9) Conduct and document all staffings. 10) Notify ISC of requests by individual to adjust/amend PCP or Implementation Strategy. 11) Analyze Outcomes and submit monthly Q notes. 12) Demonstrate knowledge of Agency policies and procedures. 13) Assist DSPs in the execution of CDS activities, which can include classroom and community
Job Duties:
1) Train staff to fulfill documentation, programming and training requirements to satisfy finders and external accrediting bodies. 2) Oversee all programming, planning, curriculum development and coverage. 3) Oversee all outcome tracking to ensure accuracy. 4) Maintain CDS caseload files assigned 5) Committee participation as assigned. 6) Other duties as assigned.

$22.75 per hour
