Job Description

Essential Functions, Roles, and Responsibilities Develop and lead small, medium, and large group activities.

Contribute to residents' care planning: assessments, care plan goals and approaches, and progress notes as well as attend care conference meetings.

Assist with documentation: daily attendance records, precautions list, one-to-one records, care plan changes, etc.

Prepare rooms, equipment, and supplies prior to each activity, and clean up after each activity.

Maintain safe activity environment by using appropriate risk management, preventive maintenance, cleaning, and storage.

Assist Activity Director in evaluating effectiveness of activities.

Conduct one-to-one programs.

Transport residents to and from activities as needed.

Assist with field trips and outings.

Assist with media correspondence, mailings, newsletters, bulletin boards and displays.

Proactively encourage and support residents' rights.

Work with residents who are low functioning. Other Duties Assist the Activity Director in the planning, development, and implementation of activities that enrich the lives of the residents.

Assist with development of the monthly calendar, including providing creative input, ideas, and suggestions for programs and volunteer recruitment.

Assist Activity Director in evaluating effectiveness of program, individual needs, and participation levels of each resident.

Work with volunteers - assist in work direction of volunteers as needed.

Follow established chain of command concerning department and facility business.

Encourage resident and family participation in activities.
