SouthernCare, a Gentiva company, is seeking a Hospice Registered Nurse to provide compassionate end-of-life care to patients in the Tuscaloosa area. In this role, you will manage a caseload of hospice patients, conduct comprehensive assessments, develop and update individualized care plans, and administer medications and treatments focused on comfort and symptom management. You will coordinate closely with physicians, social workers, chaplains, and aides, ensuring seamless, patient-centered care. The Hospice RN also educates and supports families, documents care accurately, and participates in on-call rotation to maintain high-quality, responsive hospice services. Responsibilities Provide compassionate hospice nursing care focused on comfort and symptom management Perform comprehensive patient assessments and ongoing evaluations Develop, implement, and update individualized hospice care plans Administer medications, treatments, and interventions per physician orders Educate and support patients and families on disease process, hospice philosophy, and care options Collaborate with interdisciplinary team members including physicians, social workers, chaplains, and aides Document all care, assessments, and communications accurately and timely in the medical record Participate in on-call rotation and respond to urgent patient needs Coordinate care transitions and communicate changes in condition to providers and family Ensure compliance with hospice regulations, quality standards, and safety protocols Required Skills Hospice and palliative care nursing Patient assessment and care planning Pain and symptom management IV and medication administration Wound care Care coordination and case management Electronic health record (EHR) documentation Patient and family education Crisis intervention and de-escalation Regulatory and compliance knowledge (hospice)
