At Elmore Assisted Living, we're looking for an experienced RN who is comfortable being a trusted clinical resource for residents, families, and the caregiving team. This isn't about completing a list of tasks and moving on to the next patient. It's an opportunity to use your assessment skills, clinical judgment, communication, and nursing experience to help determine what residents actually need. Your Experience Should Count for Something When a caregiver tells you: "She's just not acting like herself tonight." There may not be an order telling you exactly what to do next. That's where your nursing judgment comes in. You'll assess the resident. Review what's changed. Consider medications, diagnoses, recent events, vital signs, symptoms, and baseline status. Then you'll determine the appropriate next step and coordinate with the right people. Sometimes that means continued monitoring. Sometimes it means contacting a provider. Sometimes it means involving family, pharmacy, home health, hospice, or another healthcare partner. And sometimes it means recognizing that a resident needs a higher level of care. Be the Clinical Resource Your Team Trusts Our caregivers spend a tremendous amount of time with residents. They notice things. A resident isn't eating. Someone seems more confused. There's a change in mobility. A medication doesn't seem to be having the expected effect. A family member has a concern. You're the nurse they turn to. You'll help caregivers understand what to watch for, ask additional questions, assess the resident, provide direction within your scope, and determine when additional clinical intervention is necessary. You don't need to personally do everything. You need to know what needs to happen and how to help the team get there. What You'll Be Doing Perform resident assessments and respond to changes in condition . Use clinical judgment to determine appropriate nursing interventions. Administer medications and treatments as required. Develop and update individualized service plans and nursing interventions. Provide clinical oversight and guidance to caregivers and medication staff. Communicate with residents and families regarding healthcare needs. Coordinate with physicians, pharmacies, hospitals, therapists, home health, hospice, and other healthcare partners. Support admissions and transitions of care. Maintain accurate and timely clinical documentation. Help ensure compliance with Minnesota Assisted Living regulations and clinical standards . Participate in shared rotating on-call responsibilities. We're Looking for a Nurse Who Can Put the Pieces Together Your background doesn't have to look exactly the same as the RN sitting next to you. Experience from Assisted Living, Memory Care, Long-Term Care, Skilled Nursing, Home Health, Hospice, Hospital Nursing, Geriatrics, or Community-Based Care can all translate. What matters is how you think as a nurse. We're looking for someone who can: Perform a strong nursing assessment. Recognize subtle changes in condition. Prioritize competing clinical needs. Communicate confidently with providers. Provide direction to caregivers and medication staff. Know when something can be managed within the community and when it needs to be escalated. Document clearly and appropriately. Remain calm when an unexpected clinical situation develops. Understand the importance of regulatory compliance and following established clinical processes.
