Flexible shift options are available seven days a week, allowing you to create a schedule that suits your needs, with a minimum weekly or bi-weekly hourly commitment.

Responsibilities:

Conduct comprehensive home health risk assessments, averaging approximately 45 minutes per visit Evaluate patients medical history, existing conditions, and medication profiles Deliver education and guidance regarding health conditions and medication management Identify gaps in care and provide actionable recommendations No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner License in Illinois Board Certification through AANP or ANCC Current CPR (BLS) certification Master of Science in Nursing (MSN) from an accredited program At least six months of Nurse Practitioner experience (preferred)
