An opportunity is available for an experienced Nurse Practitioner to provide clinical expertise through home health risk assessments for Medicare Advantage members in Marion County. Serving communities such as Centralia, Salem, and Sandoval, you will play a key role in proactive healthcare in patients homes. This per diem position offers the flexibility to design your own work schedule, with shifts available every day of the week.

Schedule:

Flexible shifts available Monday through Sunday Create your own schedule Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

Conduct annual in-home health risk assessments on Medicare Advantage plan members Review and evaluate prior and current medical history, existing medical conditions, and medication regimen Educate patients on their health conditions and prescribed medications Identify gaps in care and recommend next steps to resolve them Visits are typically 45 minutes and may include annual assessments or post-discharge follow-ups No requirement for development of treatment plans or medication prescribing

Qualifications:

Active Nurse Practitioner license in Illinois Board Certification through AANP or ANCC Graduate of an accredited MSN program 6+ months of current Nurse Practitioner experience preferred Current BLS/CPR certification
