Yes, mileage reimbursed between patient visits (from first patient to last patient) Position Summary We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our dedicated home health team serving patients throughout Shepherdsville, Bullitt County, and Jefferson County, Kentucky. This role is responsible for delivering high-quality nursing care in patients' homes, promoting positive health outcomes, and ensuring compliance with all clinical and regulatory standards. The ideal candidate is organized, patient-focused, and experienced in clinical assessment, care coordination, and documentation within a home health setting. Key Responsibilities Provide skilled nursing services to patients in their homes according to physician orders and established care plans. Complete Start of Care (SOC) admissions, comprehensive assessments, and ongoing patient evaluations. Develop, implement, and update individualized care plans based on patient needs and clinical findings. Monitor patient progress, identify changes in condition, and communicate recommendations to physicians and the interdisciplinary care team. Administer medications, treatments, wound care, and other skilled nursing interventions as ordered. Accurately document all patient encounters, assessments, treatments, and care coordination activities in a timely manner. Coordinate care with physicians, therapists, social workers, and other healthcare professionals to ensure optimal patient outcomes. Educate patients and caregivers on disease management, medications, safety, and preventive health measures. Ensure compliance with Medicare guidelines, agency policies, and state and federal regulations. Participate in on-call coverage, including one weekend per month. Qualifications Current, unrestricted Registered Nurse (RN) license in Kentucky. Graduate of an accredited nursing program. Minimum of one year of nursing experience required; home health experience preferred. Strong assessment, clinical decision-making, and patient education skills. Excellent communication and organizational abilities. Ability to work independently while collaborating effectively with a multidisciplinary team. Current driver's license, reliable transportation, and auto insurance required.
