Delivering essential care to patients in their homes, this per diem opportunity focuses on conducting home health risk assessments for Medicare Advantage members throughout Calcasieu County, Louisianaincluding Lake Charles, Sulphur, and DeQuincy. As a Nurse Practitioner in this role, you will play a key part in supporting patient outcomes by completing thorough clinical and risk assessments, while enjoying the flexibility to design your own schedule.

Schedule:

Flexible shifts available Monday through Sunday Self-scheduling for maximum autonomy Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

Perform annual risk assessments during in-home visits (approximately 45 minutes each) Review and evaluate patients medical history, conditions, and medication use Provide patient education regarding diagnoses and medications Identify gaps in care and offer recommendations to resolve them No requirement to prescribe medications or establish ongoing treatment plans

Qualifications:

Active Nurse Practitioner license in Louisiana Board Certification through AANP or ANCC Completion of MSN from an accredited nursing program Current CPR certification (BLS) At least 6 months of recent Nurse Practitioner experience preferred Adaptable and comfortable working at various locations throughout
