Key Responsibilities:

Perform annual and post-discharge risk assessments for Medicare Advantage members within Calcasieu County Assess patients' medical and medication histories Educate individuals on their medical conditions and care regimens Identify care gaps and make recommendations for addressing them This role involves evaluation onlyno treatment planning or prescribing

Qualifications:
