Home Health Care, Inc. is looking for an experienced Registered Nurse (RN) to complete home health admissions throughout the Northern Greater Twin Cities Metro Area . If you're an RN who knows how to walk into a new patient's home, conduct a thorough assessment, complete an accurate OASIS, identify the patient's needs, and establish a strong clinical foundation for the team that follows, we want to talk to you. Your Assessment Sets Everything in Motion A strong Start of Care is more than documentation. It's your opportunity to understand why the patient was referred, what changed clinically, what medications they're actually taking, what their home environment looks like, what support they have, where the risks are, and what needs to happen next. Then you bring those pieces together. Assess. Identify. Educate. Plan. Communicate. When you do it well, the Case Manager and interdisciplinary team can move forward with a much clearer understanding of the patient. Their medications changed three times. They have new equipment in the home. The family has questions. The referral contains one version of the story, but your assessment uncovers additional clinical needs. That's why an experienced admissions nurse matters. You're not checking boxes. You're using your nursing experience to determine what's actually happening and helping create a plan that makes sense for the patient in front of you. What You'll Be Doing Complete comprehensive Start of Care assessments for new home health patients Perform accurate and timely OASIS documentation Complete medication reconciliation and identify potential concerns Assess clinical, functional, safety, and environmental needs Develop individualized plans of care based on patient needs and goals Educate patients and caregivers on medications, safety, disease management, and home health expectations Identify immediate clinical concerns and communicate appropriately Coordinate with physicians and referring providers Collaborate with RN Case Managers, PTs, OTs, SLPs, social workers, and other clinicians Ensure the ongoing care team has the information needed to move forward confidently Maintain accurate and compliant clinical documentation This Role Is for the RN Who Likes the Beginning of the Puzzle New patient. What You Bring Required:
Active Minnesota Registered Nurse (RN) license Experience completing Start of Care assessments Working knowledge of OASIS Strong head-to-toe assessment and critical-thinking skills Strong clinical documentation skills Ability to independently evaluate new patients in their homes Excellent patient, caregiver, and provider communication Valid driver's license and reliable transportation
Preferred:
Previous home health nursing experience Strong OASIS proficiency Home health admissions experience Experience with medically complex or post-acute patients Start Strong.
