Job Description

Well Care Home Health is seeking a Home Health Registered Nurse to deliver compassionate, one-on-one care to patients in their homes across North Carolina. In this role, you will perform comprehensive assessments, provide skilled nursing services such as wound care, IV therapy, and medication management, and develop individualized care plans in collaboration with physicians and an interdisciplinary team. You will educate patients and families on disease management and safety, document care in the EMR, and coordinate services to support recovery, chronic condition management, and independence. Responsibilities Provide skilled nursing care to home health patients including assessments, wound care, IV therapy, and medication management Develop, implement, and evaluate individualized care plans in collaboration with physicians and the interdisciplinary team Educate patients and families on disease management, medications, and home safety to promote independence Document all patient care, observations, and interventions accurately and timely in the EMRCoordinate care and communicate effectively with therapists, aides, and other care providers Monitor patient progress, recognize changes in condition, and take appropriate clinical action Ensure compliance with agency policies, regulatory standards, and infection control practices Participate in case conferences, staff meetings, and ongoing clinical training Required Skills Skilled nursing assessment Wound care management IV therapy and medication administration Care planning and case management Electronic medical record (EMR) documentation Patient and family education OASIS documentation Infection control practices Time management and prioritization Interdisciplinary team collaboration
