Summary:
Life Connections Specialized Support Serviceshas implemented a Nursing Consultation & Training Department in which we will provide and support Area Agencies and other provider agencies with Nursing needs. The contracted Nurse Trainer will maintain consistent communication with Life Connection's Director of Nursing regarding nursing services provided, clinical findings, recommendations, concerns and any follow-up needs to ensure timely coordination and continuity of nursing services.
Minimum Qualifications:
RN in New Hampshire 1 year of RN experience in the past 5 years Ability to become a Nurse Trainer (6 hour course) in the first 60 days Valid driver's license Authorized to work in the US.
Roles & Responsibilities:
The specific roles of the Nurse Contractor may include, but are not limited to: Provide professional nursing guidance based on the specific needs of the organization Review and assess nursing related needs pursuant to the organization's policies and NH regulations Serves as liaison to/for health care practitioners Consult with the organization's leadership for nursing concerns Assess and monitor client health care needs and make appropriate referrals if needed Conducts nursing assessments including preparation of care plans Provides nursing on call services as scheduled and as needed Certifies staff in medication administration in accordance with organization and state regulations Oversee residential nursing practices and ensure compliance with applicable nursing care regulations and practice standards in conjunction with Residential Program Director Provides direct nursing care as needed Works efficiently and in cooperation with other team members regarding issues of continuity of care, quality support, and individual areas of concern. Conducts Nursing QAs in accordance with organization and state regulations Develops, implements and evaluates progress towards achieving annual health care goals for clients Conducts in-service training and workshops for staff regarding health and medical issues. Conducts 5/30 Evaluations when a client moves Aid in and/or complete nursing portion of CERTs Provide hands on medical training for client specific needs Conduct Self Medication Administration Assessments Other duties as assigned by DON, Area Agency and/or Provider Agency LifeConnections Specialized Support Services assures equal opportunity and public access and, if necessary, affirmative action in all its policies regarding employment and provision of services.
