- 2 visit per week | North country-Essex NY Earn $55/hr + $90 Weekly Mileage Reimbursement North country

3 Weeks Remote Training 1 Week Field Shadowing Your Role as anRN Nurse Assessor:
Conduct in-home patient assessments (2 visit per week) Complete UAS-NY assessments accurately Evaluate medical, functional & care needs Coordinate with interdisciplinary healthcare teams Help patients access essential long-term care servicesRequirements:
