Home Health Nurse Assessor | Per diem
2 visit per week | North country-Franklin county NY Earn $55/hr + $90 Weekly Mileage Reimbursement North country

Available Paid Structured Training Program:

3 Weeks Remote Training 1 Week Field Shadowing Your Role as an

RN Nurse Assessor:

Conduct in-home patient assessments (2 visit per week) Complete UAS-NY assessments accurately Evaluate medical, functional & care needs Coordinate with interdisciplinary healthcare teams Help patients access essential long-term care services

Requirements:
