This full-time, 13-week contract offers a Monday through Friday schedule from 8:00 AM to 5:00 PM, providing consistency while making a positive impact on patient outcomes. Youll conduct 3 to 5 patient visits daily (targeting roughly 30 points per week) and be reimbursed for mileage traveled within your assigned territory.

PM Key Responsibilities:

Complete comprehensive OASIS assessments and formulate individualized care plans.

Deliver skilled nursing services:

wound care, IV therapy, medication administration, and chronic disease management. Monitor patient progress, recognize changes in condition, and communicate with the care team and physicians. Educate patients and families about illnesses, treatment plans, medications, and preventive care strategies. Document care efficiently and accurately using Home Care Home Base (HCHB). Collaborate with physical therapists, occupational therapists, social workers, and home health aides for coordinated patient care. Attend case conferences, staff meetings, and participate in ongoing education. Maintain compliance with regulatory and agency standards for care delivery.

Qualifications:

Active RN license in good standing in the state of practice. At least one year of recent Home Health experience (field experience highly preferred). Proficiency in Home Care Home Base (HCHB) is required. Strong familiarity with OASIS documentation tools. Must possess reliable transportation, a valid drivers license, and current auto insurance.
