Role Qualifications:

A licensed nurse- LPN or RN with excellent communication skills, proficiency with documentation, time management, positive interpersonal relations, be an advocate for patient safety and access to quality healthcare, and be a moral compass to ensure all aspects of the profession are met with honesty and integrity Position responsibilities:

Greets new admissions and family members. Welcomes them to Uplands. Demonstrates positive customer relations. Provides directions within facility to patients/residents or family members for locations of areas/departments, etc. to facilitate comfort and integration. Communicates positively, cooperates, and assists physicians as needed. Demonstrates positive employee relations. Communicates positively (verbally and non-verbally) with other departments. Demonstrates flexibility in handling and constantly changing environment. Extend warmth and acceptance to each other and to all I serve by being courteous in greeting, positive in attitude, supportive of co-workers and team-members, and praising others for doing a good job.

Justice:

Take responsibility for my own actions in my work, in my use of time, and in keeping my word when I make promises. Maintain respectful working relationships by communicating openly and honestly with coworkers. Maintain the same standards for all patient/residents, their families and my co-workers. Work collaboratively with Nurses, Nurse Managers, and other departments on admission and discharge processes. Remains calm while facing challenges of daily nursing activities. Stewardship:

Facilitates admissions by obtaining required paperwork. Complete all necessary and required forms / documentation in a timely manner. Reviews/receives report on condition of patient at time of admission Completes admission interviews with new patients and inputs information into the computerized medical record or paper form/record. Observes, and records symptoms and conditions of patients/residents as required for the assessment process. Informs and updates unit nurse and nursing assistant as to patient condition and plan of care during the admission process. Checks orders on admissions. Cleans up incomplete admissions clip boards (hold admissions forms, etc.). Creates initial temporary care plans on new patients. Provides professional nursing services consistent with the RN/ LPN job description, philosophy, policies and procedures of the facility Assists with discharge planning as Setting up DME etc.. Assists Charge Nurse and Nurse Managers as needed. Report to work on time and complete work assignment on time, while not overextending breaks and/or lunch. Performs direct patient care, as assigned, if necessary, due to inability to replace an employee due to illness or vacancy. Attends regular staff meetings, in-services.
