In this role, you will deliver skilled nursing care to medically complex adult patients in their homes, including assessments, medication administration, wound and IV care, and monitoring high-acuity conditions. You will develop and update care plans, coordinate with physicians and therapists, and educate patients and families. Responsibilities Provide skilled nursing care to adult high-acuity home health patients Conduct comprehensive assessments and monitor complex clinical conditions Administer medications, treatments, wound care, and IV therapy as ordered Develop, implement, and update individualized care plans Document all care and patient responses in the EMR accurately and timely Educate patients and families on disease management, medications, and equipment Coordinate care and communicate with physicians, therapists, and other team members Follow infection control, safety, and regulatory standards in the home setting Participate in case conferences and quality improvement initiatives Support Aveanna's patient-first, compassionate care culture Required Skills Adult home health nursing High-acuity patient management Care planning and care coordination IV therapy and wound care Medication administration and reconciliation Electronic medical record (EMR) documentation Patient and family education Vital signs and clinical assessment Infection control practices Interdisciplinary team collaboration
