Job Description

Liberty Health Care Center is seeking an experienced and detail-oriented RN or LVN to serve as our Wound Care Treatment Nurse. This is an important clinical role focused on wound treatment, pressure injury prevention, accurate documentation, and interdisciplinary coordination. Competitive compensation based on experience Full-time position with consistent scheduling Medical, dental, and vision insurance 401(k) with employer contributions Paid time off Company-paid life insurance Daily Pay available Supportive nursing leadership Collaborative interdisciplinary care team Opportunity to make a direct impact on resident outcomes Key Responsibilities Administer wound treatments according to physician orders and facility policies Lead pressure injury prevention and skin-integrity initiatives Conduct weekly skin assessments and complete required wound documentation Track and trend wound-healing progress and relevant infection-control data Participate in wound rounds and interdisciplinary care-plan meetings Maintain accurate wound logs and required compliance documentation Coordinate care with physicians, nurse practitioners, consultants, and other clinical professionals Educate nursing staff, residents, and families regarding wound-care plans and prevention strategies Ensure accurate clinical documentation and transcription of treatment orders Assist with resident admissions, transfers, and discharges as needed Administer medications and provide skilled nursing services within the nurse's scope of practice Support change-of-condition reporting and appropriate follow-up communication Participate in the nursing on-call rotation as assigned Maintain compliance with state, federal, professional, and facility standards Qualifications Active, unencumbered Texas RN or LVN license Current CPR certification Skilled nursing, rehabilitation, or long-term care experience preferred Previous wound-care or treatment-nurse experience is required Wound-care certification is a plus but not required Strong knowledge of wound care, skin integrity, and pressure injury prevention protocols Understanding of state and federal long-term care regulations Excellent clinical documentation and analytical skills Ability to work independently and make sound clinical decisions Strong communication, organization, and follow-through Ability to stand, walk, bend, and lift up to 60-70 pounds as required Ability to pass required background screening and drug testing and meet applicable health requirements What Sets You Apart You are detail-oriented and compliance-driven You communicate effectively with interdisciplinary team members You stay organized while managing multiple priorities You are comfortable educating staff, residents, and families You take a proactive approach to preventing avoidable skin issues You consistently follow through on treatments, documentation, and changes in condition Make a Meaningful Clinical Impact As the Wound Care Treatment Nurse, you will play a vital role in promoting healing, preventing complications, and protecting residents' comfort and quality of life.
