The Impact You'll Make As an RN in our IDDW and community-based services team, you'll be more than a clinician—you'll be a trusted advocate, educator, and problem-solver. Your expertise will help individuals with intellectual and developmental disabilities live safely and meaningfully in their homes and communities. From developing personalized care plans to training direct support staff, your work will directly shape the quality of life for the people we serve. What You'll Do Conduct comprehensive nursing assessments for individuals enrolled in the IDDW program, ensuring compliance with state and federal guidelines. Develop, implement, and monitor individualized health care and risk management plans. Oversee medication management protocols. Provide health education and training to direct support professionals, families, and caregivers. Collaborate with interdisciplinary teams, including case managers, therapists, and physicians—to coordinate holistic care. Monitor and document health status changes, ensuring timely interventions. Advocate for individuals' rights, safety, and access to community resources. Participate in quality assurance activities and maintain accurate, timely documentation in accordance with IDDW policy. What You'll Bring Current, unrestricted RN license in the state of West Virginia. Experience in IDD, community health, home health, or related nursing fields preferred. Strong knowledge of person-centered care principles and IDDW program requirements. Excellent communication skills for working with individuals, families, and multidisciplinary teams. Ability to work independently in community settings while maintaining strong organizational and documentation skills.
