This opportunity offers a rewarding role for an experienced Registered Nurse specializing in Home Health. You'll work directly with patients in their homes, delivering high-quality care and making a meaningful impact in the community. Schedule:

Monday Friday, 8:00 AM 5:00 PM Full-time, 13-week contract

Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized care plans. Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management. Monitor patient progress, recognize changes in condition, and communicate all updates with the care team and physicians. Educate patients and families on treatment plans, medications, and preventive healthcare practices. Ensure accurate, prompt documentation using the Home Care Home Base (HCHB) system. Collaborate closely with physical therapists, occupational therapists, home health aides, and medical social workers for coordinated care. Participate in case conferences, staff meetings, and ongoing professional education sessions. Maintain compliance with all regulatory and agency documentation and patient care standards.

Qualifications:

Active RN license in good standing for the state of practice At least 1 year of recent home health experience (field work preferred) Strong proficiency with Home Care Home Base (HCHB) and OASIS Reliable transportation, valid drivers license, and auto insurance
