This is a full-time, in-person office position in Hacienda Heights, CA. We are looking for a candidate with prior Home Health QA experience who can independently review clinical documentation, identify documentation or compliance issues, and handle routine QA responsibilities with minimal training. The ideal candidate is familiar with Home Health clinical records, physician orders, Plans of Care, nursing and therapy documentation, and Medicare-related documentation requirements. Responsibilities Independently perform QA review of Home Health clinical documentation Review Plans of Care, physician orders, nursing notes, therapy notes, and other clinical records for accuracy and completeness Identify missing signatures, incomplete documentation, inconsistencies, and potential compliance issues Follow up with nurses, therapists, physicians, and office staff regarding corrections or missing documentation Support new patient intake and referral review Review referral documents, insurance information, and clinical records before admission Assist with patient admission coordination and scheduling Communicate with physicians, nurses, therapists, hospitals, pharmacies, laboratories, DME providers, and other healthcare partners as needed Maintain organized and accurate electronic patient records Help keep clinical records survey-ready and audit-ready Support daily clinical and administrative office operations Qualifications Current and active California RN or LVN license required Previous Home Health QA experience required Hands-on experience reviewing Home Health clinical documentation Familiarity with Plan of Care, physician orders, nursing notes, therapy notes, and QA review Knowledge of Medicare Home Health documentation and compliance requirements Experience identifying incomplete, inconsistent, or non-compliant documentation OASIS review experience preferred Home Health intake or clinical coordination experience preferred Strong spoken and written English required Spanish-speaking preferred Strong computer and organizational skills Excellent attention to detail Ability to work independently and communicate professionally with physicians, clinicians, patients, and office staff Position Details Full-time In-person / office-based Monday through Friday Approximately 40 hours per week

Pay:

$45.00 per hour
