RN Hospital Liaison for Inpatient Hospice Acute Care Units Full-Time

Job Description

JOB SUMMARY
America's first hospice, The Connecticut Hospice, Inc., is a 503 (c) not for profit organization, accredited and certified by The Joint Commission (TJC) as the first palliative hospital in the US., and has an immediate opening available in their Central Intake & Admissions Department for a full-time RN Liaison. The RN Liaison is responsible for coordinating the transition of new and existing patients back to or into, the inpatient hospice hospital or home care setting for hospice or can support services. Provides education specific to the patient's diagnosis, reason for admission and goals. Functions as a resource person to community agencies. Local hospital care coordinators, discharge planners, social workers, patient and family throughout the transfer process to either home care or inpatient. Coordinates this transfer with all parties for a smooth transition. Answers any further questions as necessary for a comprehensive plan of care. Provides for coverage within the CIA Department on a routine and as needed basis. Performs duties at either the CIA office or in the field as a Liaison RN at specific facilities. Communicates with insurance companies for authorization of initial nursing visits whenever the Finance Department is unable to do so for home care patients. For HIP patients, immediately communicates with insurance company for an initial authorization and for timely updates. When acting as the Intake RN and Care Coordinator, initiates the process to obtain the insurance authorization for HIP patients and documents follow-up as needed. Obtains follow-up clinical information as needed. Participates in orientation of new employees, staff education, family meetings and direct patient care. Provides on-site and off-site supportive services to CIA Department to facilitate transfer of patients and cover staffing within the CIA. This position is "hands on". Must support the Connecticut Hospices' vision, mission, and operating principles.

EDUCATION AND EXPERIENCE
Must have active Registered Nurse (RN) in the State of Connecticut Graduate of an accredited school of nursing BSN preferred Hospice and Palliative Home Care nursing experience Two to three years experience in a clinical healthcare role Knowledge of current nursing, medical and health trends and practices in rendering of patient care Knowledge of referral procedures, criteria and admission guidelines The Connecticut Hospice, Inc. is an Equal Opportunity Employer.
