Your visits may include:

Comprehensive nursing assessments Wound assessment and wound care Medication reconciliation and management Injections and other skilled nursing procedures Monitoring acute and chronic medical conditions Patient and caregiver education Identifying and communicating changes in condition Coordinating with physicians and other providers Collaborating with PT, OT, SLP, social work, and other members of the care team Helping prevent avoidable emergency room visits and hospitalizations Completing timely documentation in Homecare Homebase (HCHB) You aren't simply checking a task off a list.
