JOB DESCRIPTION

The Intake Coordinator is responsible for coordinating the end-to-end intake and onboarding process for both clients and Direct Support Professionals (DSPs), from referral or application acceptance through readiness to begin services. This role ensures timely eligibility verification, documentation collection, onboarding coordination, and communication across internal teams and external partners. The Intake Coordinator works closely with Community Outreach Coordinators, Human Resources, Case Management, Area Directors, and Operations to align client service starts with available caregiver capacity, maintain complete and compliant records, and support smooth, timely service delivery. This position plays a critical role in balancing client demand with caregiver readiness while ensuring all intake and onboarding processes meet agency, ODHS, and ODDS requirements. This role does not provide direct services and does not manage cases.

RESPONSIBILITIES 1.

Client Intake Coordination Receive warm handoffs from Client Relations Representatives and referral partners. Schedule and coordinate intake meetings with clients, families, and internal teams. Track intake progress from referral through service start. Flag delays, barriers, or intake issues and escalate as appropriate. Ensure families and representatives understand next steps in the intake process. 2. DSP Intake Coordination Receive warm handoffs from Community Outreach Coordinators following caregiver application. Schedule and coordinate onboarding steps, including orientation, training, and required appointments. Track DSP onboarding progress from offer acceptance through readiness to start services. Flag delays, barriers, or onboarding issues and escalate as appropriate. Ensure caregivers understand onboarding requirements, timelines, and next steps. 3. Eligibility & Authorization Verification Verify client eligibility through CDDPs, brokerages, and applicable ODDS systems. Confirm authorized services, approved hours, and projected start dates. Verify completion of required DSP pre-employment and onboarding requirements. Confirm caregiver readiness to begin services. Communicate eligibility and readiness timelines to Area Directors and Operations. 4. Documentation & Records Management Collect, review, and process all required client and caregiver intake documentation. Enter and maintain accurate, up-to-date client records in internal systems. Ensure all documentation meets ODHS and ODDS requirements. Maintain complete, accurate, and audit-ready client files. Support audits, compliance reviews, and documentation requests as needed. 5. Service Matching & Capacity Coordination Coordinate with Area Directors and Operations to align new client intakes with available DSP capacity. Support matching of clients and caregivers based on service needs, hours, location, and scheduling. Identify and flag capacity constraints that may impact service starts. 6.

Collaboration:

Collaborate closely with Area Directors and Operations. Communicate intake demand trends and capacity needs to the Director of Operations. Align client intake flow with caregiver availability. 7.

Performance Metrics:

Timeliness and accuracy of completed intakes. Intake and onboarding to service start timelines. Documentation compliance and audit readiness. Alignment of caregiver hiring with service demand and capacity. 8.

Community Events/Involvement:

Actively engage in community involvement initiatives as needed. Assist in planning community events as needed. Attend company events within proximity as needed. Volunteer - minimum of 4 hours, maximum of 8 hours of mandatory paid volunteer hours per year. 9.

General Responsibilities:

Create quarterly newsletter content. Stay informed on ODDS updates, policies, procedures, and transmittals. Reviews task schedules and submit for approval as needed. Responsible for being on call one weekend a month. General office management tasks as required.
