POSITION SUMMARY
: The primary purpose of your job position is to assist the Director of Community Liaison in planning, developing, organizing, implementing, evaluating, and directing social service programs as they relate to the admissions and intake processes and activities of new patients, in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that medically related emotional and social needs of the resident are met/maintained on an individual basis. This includes patient and family education and orientation before, during, and after admission, counseling and problem resolution as needed, and inter-departmental coordination of same. It also includes responsibilities for community outreach and education as outlined below, and as needed at the direction of your supervisor. This is a staff level position with reporting responsibility to the Director Community Liaison.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Assist the Director of Community Liaison in conducting outreach and education on facility services and availability of patient care services to professional contacts, such as physicians, hospitals, related medical professionals, and discharge planners. Assist the Director of Community Liaison in conducting outreach and education on facility services and availability of patient care services to professional agencies and organizations, such as public health agencies, senior citizen groups, organizations serving disabled persons, and state, county, and local medical societies, or other similar groups and institutions. $15.00•$17.00 per hour
