Job Description

Overview 1st Texas Home Health is seeking a dynamic and compassionate Home Health Intake Coordinator to join our healthcare team. In this vital role, you will serve as the first point of contact for patients and their families, coordinating essential intake processes across various medical settings. Your expertise will ensure seamless communication, accurate data collection, and efficient management of patient information, all while providing exceptional service in a fast-paced environment. This position offers an exciting opportunity to make a meaningful impact on patient care and support multidisciplinary teams in delivering high-quality healthcare services. Responsibilities Act as the primary liaison for patient intake, gathering comprehensive medical histories, social information, and insurance details using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems. Coordinate with hospital departments such as social work, case management, and behavioral health to facilitate smooth discharge planning and patient transitions. Verify insurance coverage, including health insurance knowledge and managed care processes, to ensure appropriate authorization for services. Conduct initial assessments for patients requiring specialized care such as critical care, ICU, pediatrics, or hospice services. Collaborate with multidisciplinary teams to facilitate utilization review and utilization management processes in compliance with HIPAA regulations. Maintain detailed documentation of patient interactions, ensuring accuracy in CPT coding, ICD-9/10 coding, and medical terminology for billing and reporting purposes. Support patients from diverse cultural backgrounds through effective communication and bilingual assistance when applicable. Qualifications Proven experience in hospital settings with a focus on intake coordination or case management. Strong background in critical care, acute care, ICU, emergency medicine, or hospital medicine environments. Knowledge of medical coding systems including
CPT, ICD-9/10
coding; familiarity with discharge planning and utilization review is preferred. Experience working with EMR/EHR systems and proficiency in Microsoft Office applications. Excellent client communication skills with the ability to handle sensitive information discreetly under HIPAA guidelines. Ability to work effectively with individuals from diverse cultural backgrounds and provide compassionate social services support. Pay:
$19.00 - $25.00 per hour Expected hours: 40.0 per week
