Job Description
Solaris HealthCare is seeking a compassionate Speech Language Pathologist to join our Healthcare & Medical Services team in our skilled nursing and rehabilitation centers. In this on-site role, you will assess and treat residents with speech, language, cognitive-communication, and swallowing disorders, creating evidence-based treatment plans that promote safety, function, and dignity. You'll collaborate closely with nursing, rehab, and medical teams, educate families and staff, and maintain accurate clinical documentation. Responsibilities Evaluate residents' speech, language, cognitive-communication, and swallowing abilities using standardized assessments. Develop and implement individualized treatment plans to improve communication and safe swallowing. Provide one-on-one and group therapy in a skilled nursing and rehabilitation setting. Collaborate closely with nursing, rehab, and medical teams to coordinate care and optimize functional outcomes. Educate residents, families, and staff on communication strategies and dysphagia management. Document evaluations, progress notes, and care plans in accordance with regulatory and facility standards. Participate in care conferences and contribute to discharge planning and goal setting. Adhere to high clinical standards, ethical guidelines, and patient confidentiality requirements. Support a patient-centered, compassionate environment that promotes dignity and respect. Engage in ongoing professional development and training offered by Solaris Health Care.
