Qualifications Speech-Language Pathology License Full Job Description Role Overview:
A Home Health Speech Language Pathologist provides personalized speech, voice, language, hearing, and swallowing therapy in patients' homes or facilities. They evaluate, develop, and review treatment plans, administer diagnostic tests, and monitor patient progress. The role includes instructing patients and families on communication strategies, maintaining documentation, and participating in care coordination and discharge planning.
Responsibilities:
Evaluate and treat speech/language disorders Develop and review treatment plans Administer diagnostic tests and interpret results Monitor and report patient condition changes Train patients in communication aids and strategies Provide education to patients, families, and healthcare staff Maintain accurate documentation and reports Participate in care coordination and discharge planning Ensure confidentiality and professional conduct
Qualifications & Skills:
Minimum six months of experience as a speech therapist Current, unrestricted license and CPR certification Valid driver's license, auto insurance, reliable transportation
Preferred:
Home health experience
Additional Details:
