About the Opportunity We are currently seeking a Speech-Language Pathologist / Speech Therapist to provide home health services throughout the Worcester Area.
Please note:
This position requires the ability to travel to multiple patient locations and provide care in patients' homes.
Qualifications:
Qualifications Meets ASHA Certificate of Clinical Competence (CCC) requirements. Minimum 6 months of experience as an SLP/Speech Therapist. Current, unrestricted Massachusetts SLP license . Current CPR certification . Home Health experience preferred, but not required. Strong communication, organizational, and problem-solving skills . Ability to work independently, manage time effectively, and travel to patients' homes . Key Responsibilities As a Home Health Speech-Language Pathologist (SLP) , you will: Evaluate and provide speech, language, cognitive, voice, and swallowing therapy in patients' homes or facilities. Develop and regularly update the Plan of Care (POC) and treatment plans. Perform assessments and administer appropriate diagnostic and therapeutic interventions . Monitor patient progress and report changes in condition to the supervisor and/or physician. Educate patients, families, and caregivers on treatment plans, communication strategies, and safe swallowing techniques. Provide alternative communication strategies when appropriate. Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements. Participate in care coordination, discharge planning, and interdisciplinary team activities . Maintain patient confidentiality and comply with all applicable professional, company, and regulatory standards . Participate in required staff training and continuing education .
