Overview
Job Summary

Evaluates and assesses patients to establish functional, measurable goals to promote restoration of functional abilities. Provides treatment according to a written plan of care to meet established goals. Plans for discharge andmunicates findings and progress according to professional licensure/certification, scope of practice and departmental/discipline policies and procedures. Produces other reports as needed. May supervise clinical students and support staff including certified/licensed assistants, technicians, and volunteers. Ads to the ASHA Code of Ethics. Provides a caring relationship that facilitates health, and healing. Performs other duties as assigned.

Job Responsibilities

  • Evaluates and assesses the patient based on subjective and objective information.
  • Communicates results of the evaluation to patient and other caregivers.
  • Provides treatment according to established goals on plan of care.
  • Documents interventions/care in the patient's medical record.
  • Evaluates effectiveness of plan of care and reassesses patient's functional status as needed.
  • Provides discharge planning information which includes patient/family education, follow-up, and referral tomunity resources.
  • Contributes to the development of staff and students.
  • Participates in activities designed to improve health care delivery.
  • Completes assigned goals.

Minimum Required Experience

Entry level.

Minimum Required Special Skills

Basicputer literacy skills such as keyboarding, using a mouse, and navigating sites.

Minimum Required Licensure

SLP-CCC;

BLS CERTIFICATION WITHIN 14 DAYS OF HIRE

; SLP
