Job Description

Job Type Full-time Description Plaza Community Living Center is seeking a compassionate and dedicated Speech Therapist to provide high-quality speech, language, communication, and swallowing services. This position will support two facilities , providing individualized care and helping residents achieve their highest level of function, independence, and quality of life. Key Responsibilities Assess, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders. Develop and implement individualized treatment plans based on each patient's needs and goals. Conduct screenings and evaluations to identify patients who may benefit from speech therapy services. Monitor patient progress and modify treatment plans as needed. Collaborate with nursing staff, physicians, and other healthcare professionals to provide comprehensive patient care. Educate patients, residents, and families regarding communication and swallowing disorders and treatment recommendations. Maintain accurate and timely documentation in accordance with facility and regulatory requirements. Provide services across two facilities as needed. Requirements Qualifications & Skills Current Speech-Language Pathologist license/certification as required by the state. Strong clinical knowledge and assessment skills. Ability to work effectively with patients with a variety of needs and personalities. Excellent interpersonal and communication skills. Professional, compassionate, and patient-centered approach.
