Job Description

Speech Therapist PRN Deer Park Health & Rehabilitation is seeking a highly skilled and dedicated Speech Therapist PRN to join our dynamic therapy team. As a Speech Therapist PRN, you will play a vital role in evaluating, diagnosing, and treating residents with speech, language, voice, communication, and swallowing disorders. About Deer Park Health & Rehabilitation At Deer Park Health & Rehabilitation, we are committed to providing comprehensive nursing and rehabilitation services to our residents. Our team of healthcare professionals, including registered nurses, licensed practical nurses, certified nursing assistants, and therapists, work together to develop personalized plans of care to meet and exceed the recovery goals of each resident.
Key Responsibilities:
Conduct comprehensive evaluations to assess speech, language, communication, cognitive-linguistic, and swallowing abilities. Develop and implement individualized treatment plans tailored to residents' needs and goals. Provide therapy services including speech, language, voice, fluency, cognitive-communication, and swallowing interventions. Monitor and document residents' progress, making necessary adjustments to therapy plans. Collaborate with an interdisciplinary team including nursing, physical therapy, occupational therapy, and physicians to provide coordinated care. Educate residents, families, and caregivers on strategies to support communication, safe swallowing, and carryover of therapy techniques. Ensure compliance with facility policies, professional standards, and state/federal regulations.
Qualifications:
Graduate of an accredited Speech-Language Pathology program (Master's degree required). Current and valid state licensure as a Speech-Language Pathologist. Certificate of Clinical Competence (CCC-SLP) preferred. CPR certification required (or must obtain upon hire). Experience in a skilled nursing or long-term care setting preferred.
