$60.00 - $70.00 /hour Per Diem / Part-Time / Flexible Scheduling Available Join a Growing Medicare Home Health Team Nightingale Home Health is seeking a compassionate and skilled Speech Language Pathologist (SLP) to provide high-quality home health services to patients throughout the Portland Metropolitan Area. Our agency specializes in caring for Medicare patients recently discharged from hospitals and rehabilitation facilities. We are looking for clinicians who are passionate about improving communication, cognitive function, and swallowing safety while helping patients remain safely in their homes. Job Description The Speech-Language Pathologist (SLP) evaluates, diagnoses, and treats patients with communication, cognitive, and swallowing disorders in the home health setting. The SLP works closely with physicians, nurses, physical therapists, occupational therapists, and caregivers to develop individualized treatment plans that improve patient safety, independence, and quality of life. Responsibilities Evaluate and treat patients with: Speech disorders Language deficits Cognitive communication impairments Dysphagia/swallowing disorders Develop individualized treatment plans and measurable therapy goals Educate patients and caregivers regarding therapy exercises and swallowing precautions Collaborate with physicians, nurses, PTs, OTs, MSWs, and interdisciplinary team members Complete accurate and timely EMR documentation Monitor patient progress and communicate changes in condition appropriately Participate in care coordination and case conferences as needed Qualifications Required Master's Degree in Speech-Language Pathology Current Oregon SLP License Certificate of Clinical Competence (CCC-SLP) preferred Current CPR/BLS Certification Valid driver's license, reliable transportation, and auto insurance Strong communication and documentation skills Preferred Home health experience Experience with: Dysphagia management Stroke rehabilitation Neurological conditions Geriatric patient care EMR documentation experience Why Join Nightingale? ✅ Flexible scheduling ✅ Supportive interdisciplinary team ✅ Competitive compensation ✅ Opportunity to work independently ✅ Meaningful one-on-one patient care ✅ Growth opportunities in a rapidly expanding home health agency Ideal Candidate We are seeking clinicians who are: Compassionate and patient-centered Organized and dependable Comfortable working independently in the field Skilled in caregiver education and interdisciplinary communication Passionate about improving patient quality of life
