PRN Speech Language Pathologist Skilled Nursing Facility

Job Description

Job Summary:
We are seeking a licensed Speech-Language Pathologist (SLP) to join our team on a PRN (as-needed) basis in a skilled nursing facility. The SLP will evaluate and treat residents with speech, language, cognitive, and swallowing disorders to improve communication, safety, and overall quality of life.
Key Responsibilities:
Conduct comprehensive evaluations of speech, language, cognitive-communication, and swallowing disorders Develop and implement individualized treatment plans Provide therapy for dysphagia, communication deficits, and cognitive impairments Perform and interpret clinical swallow evaluations (and instrumental assessments if applicable) Monitor patient progress and adjust treatment plans as needed Complete accurate and timely documentation in compliance with state and federal regulations Educate residents, families, and caregivers on communication strategies and safe swallowing techniques Collaborate with interdisciplinary team members including nursing,
PT, and OT Qualifications:
Master's degree in Speech-Language Pathology from an accredited program Current Pennsylvania SLP license in good standing Certificate of Clinical Competence (CCC-SLP) preferred or required CPR certification Strong clinical, communication, and documentation skills
Preferred Experience:
Experience in skilled nursing, rehabilitation, or long-term care settings Experience with dysphagia management and modified diets Knowledge of Medicare Part A/B, PDPM, and SNF documentation requirements
Work Schedule:
PRN / Per Diem - flexible scheduling based on facility needs (weekday and/or weekend coverage)
Work Environment:
Skilled nursing/long-term care facility Mix of short-term rehab and long-term residents Requires standing, walking, and assisting residents as needed
