Evaluate, diagnose, and develop personalized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders. Provide evidence-based therapy aligned with physician orders and facility policies. Collaborate with an interdisciplinary team to ensure comprehensive resident care. Educate patients, families, and caregivers on treatment goals and home programs. Maintain accurate clinical documentation in compliance with regulations. Participate in care planning and discharge meetings. Qualifications include a Master's Degree in Speech-Language Pathology, current state licensure or eligibility, and CCC-SLP certification (CFY candidates welcome).
