As the Hospital Liaison, you will play a vital role in supporting patients and families as they transition from hospital to post-acute care. You will collaborate closely with hospital care management teams, physicians, and therapists to build strong relationships and drive referrals to the center, which provide unparalleled LifeWorks Rehab therapy services designed to help patients regain independence and return to their pre-illness routines. Serve as the first point of contact for patients and families making important recovery decisions. Educate patients and families on the benefits of our exclusive LifeWorks Rehab program. Collaborate with Admission Directors to ensure seamless transitions of care. Represent the company professionally within the healthcare community, demonstrating strong communication and sales skills.
