Job Description

JOB SUMMARY
Under the direct supervision of the Supportive Services Manager, the Case Manager plays a pivotal role within the Supportive Services Program at TYFS. This position focuses on delivering tailored, weekly case management services to young individuals who are actively seeking stable housing solutions. In addition to direct case management, the Case Manager organizes and facilitates life skills workshops at LTUSD, helping youths to develop essential skills for independent living. In addition, the Case Manager engages in proactive outreach by attending various community events and initiatives, effectively raising awareness about the program and connecting with potential participants who may benefit from housing services. They also take an active, hands-on role at the Drop-In Center, where they provide immediate support, resources, and guidance to youth in need, fostering a welcoming space where young individuals can feel safe and supported. This position is full-time and classified as non-exempt, offering a competitive benefits package that includes medical, dental, and vision coverage. EDUCATION & PROFESSIONAL EXPERIENCE REQUIRED FOR THE POSITION
At least two years' experience working with young adults in a community setting or possessing additional related work experience. Demonstrated ability to work effectively with difficult-to-reach service populations, especially homeless/disenfranchised youth and young adults, in a multi-disciplinary human service setting. Experience in planning and conducting individual and group activities. Excellent organizational skills and the ability to deal with multiple tasks simultaneously. Experience working with social agencies, schools, and non-traditional educational programs. Must have a reliable car and a valid, clean driver's record. Must be able to work some evenings. Must be available to be on an emergency on-call basis
RESPONSIBILITIES INCLUDE
Maintain a caseload of TLP residents from Intake and assessment to Aftercare; program activities, procedures, and policy are centered around youth voice and choice. This reflects our philosophy that only when a person is empowered can they make real change. Determine eligibility and gather intake info, and assess strengths, needs, and interests. Assist participants in entering the TLP apartment, orienting and supporting them in setting up a safe, healthy home. Provide effective case management by using a Positive Youth Development approach, combined with a harm reduction model and a trauma-informed care protocol. Develop bi-annual, monthly, and weekly Goal Plans with the youth. Assisting young people in meeting these goals to successfully transition to their own housing upon graduation from the TLP. Assist participants in identifying and assessing immediate and long-term needs, including safety, well-being, and self-sufficiency. Advocate on participants' behalf with other service providers, landlords, the legal system, etc., which could lead to providing referrals utilizing community resources. Provide skill-building in a full range of independent living skills through one-on-one and small group training. Teaching a weekly Life Skills curriculum at Mt. Tallac High and/or South Tahoe High School. Assist in health promotion areas (physical, cognitive, emotional/mental), education, and employment. Place and monitor participants in workplace apprenticeships based on their interests and skills. Supportive Services tasks include, but are not limited to: a. Aide in outreach services to youth and provide youth with information regarding programs and services available to them. b. Attend weekly Supportive meetings. c. Participate in outreach to the community regarding all aspects of the agency's programs and services. Ensure that all agencies, federal grantors, and California state mandates, policies, and procedures are followed throughout the course of work. Maintain required documentation on the program and all participants. Maintain high ethical standards when carrying out all duties and maintain strict adherence to all confidentiality, legal, and agency guidelines in all areas of service provision.
