Job Description

This position is for the southeastern area of PA. Job Summary This position is for the recruiting and initial intake of participants and caregivers for non-medical home care. You should have knowledge of the waiver enrollment process, understanding of Area on Aging, Maximus (PA enrollment broker), and support coordinators, managed care organizations (MCOs). Additionally responsibilities will include follow-up visits with participants and caregivers as needed. Duties
  • Complete assessments with new participants while meeting the hour monthly requirement
  • Complete client and employee paperwork at each meeting
  • Review and send necessary documentation to office admin at the end of each day
  • Correlate follow up issues with office admin so that proper follow up can be completed
  • Maintain well organized case notes
  • Meet with office admin to turn in necessary documentation at least once a week
  • Attend follow-up meeting with participants if needed.
  • Ensure compliance of program, services and staff with regard to appropriate regulations.
  • Initiate, monitor and provide ongoing oversight of participant service plans and eligibility requirements and individual progress in Medicaid Home and Community Based Waiver Programs to ensure timely and effective services for consumers.
  • Demonstrate ability to serve as a professional and positive representative of A+ Home Care
  • Assist in communications with
DPW/OLTL
  • Advocate for and represent participants who request support and for those who are unable or incapacitated to advocate for themselves.
  • Complete all assigned documentation a timely manner.
  • Other work-related duties as assigned. Experience
  • Must have exceptional interpersonal skills and be able to deal with differing personalities on a daily basis. Must have ability to interact with people on all levels of the organization in a professional manner.
  • Must have effective and professional communication skills
  • Must have proven organizational skills. Must be able to independently prioritize.
Participant Interaction:
must be able to maintain positive interactions with participant, their families and attendants.
  • Demonstrates the ability to utilize a wide range of appropriate knowledge, skills and abilities to achieve desired outcomes within the scope of the job responsibilities.
Must have valid drivers license and reliable transportation.
Job Type:
Full-time Pay:
$45,000.00 - $55,000.00 per year
