This paid position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those managing chronic conditions such as dementia or autism. Responsibilities Provide companionship and emotional support to clients, fostering social engagement and reducing feelings of loneliness Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by observing vital signs and noting any changes in condition or behavior Support medication management by reminding clients to take prescribed medications safely and accurately Prepare nutritious meals aligned with dietary needs and care plans while maintaining proper hygiene and cleanliness standards Use specialized equipment such as Hoyer lifts or wheelchairs to ensure safe transfers and mobility assistance Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with HIPAA regulations Follow established care plans designed by healthcare professionals to meet individual needs effectively Engage clients in meaningful activities to promote mental stimulation and emotional well-being Ensure a clean, safe environment by performing light housekeeping tasks such as cleaning and organizing living spaces Skills Proven experience in home & community care or assisted living environments; group home experience is a plus Knowledge of patient observation techniques and vital signs monitoring Familiarity with Alzheimer's care, dementia care, memory care, and working with individuals with developmental disabilities or autism Ability to assist with ADLs including bathing, dressing, grooming, meal preparation, and medication administration CPR and First Aid certification required; additional certifications such as HIPAA compliance are preferred Strong communication skills to effectively interact with clients, families, and healthcare teams Physical ability for heavy lifting and assisting with transfers using equipment like Hoyer lifts or resident lifts Experience working within long-term care settings or hospice environments is advantageous Compassionate demeanor combined with patience and flexibility to adapt to each client's unique needs Join us in creating a caring environment where every individual receives respectful support tailored to their needs.
