Maryland Behavioral Health, LLC is seeking dedicated and experienced mental health professionals to join our multidisciplinary Assisted Outpatient Treatment (AOT) Care Coordination Team (CCT). Operating under a specialized grant from the Maryland Department of Health (MDH) Behavioral Health Administration (BHA), the AOT Case Manager plays a vital, hands-on role in supporting individuals with serious and persistent mental illness (SPMI) who have experienced difficulties engaging in voluntary treatment. Travel & Driving Locations This position is highly mobile. While based out of our central offices, Case Managers must regularly travel to locate clients, conduct home-based visits, and attend court hearings across three primary regional hubs:
Baltimore City Region:
Baltimore City Circuit Courthouse and surrounding urban community settings.
Anne Arundel County Region:
Anne Arundel County Circuit Court (Annapolis) and surrounding county communities.
Eastern Shore Region:
Travel to county circuit courts and rural residential locations spanning Cecil, Kent, Queen Anne's, Caroline, Talbot, Dorchester, Wicomico, Somerset, and Worcester counties. Court Proceedings & Participation Requirements Under Maryland Senate Bill 453 (Health-General Article §10-6A), AOT is a legal, civil court process. To participate in court proceedings, the Case Manager must strictly fulfill the following mandates:
Treatment Plan Submission:
Must assist the CCT Psychiatrist in drafting and formally submitting the preliminary individualized treatment plan to the Circuit Court prior to the initial AOT petition hearing.
Collaborative Planning Requirements:
Must document that the respondent was given a "reasonable opportunity to participate" in the treatment plan's development, incorporating their preferences to the maximum extent possible.
Stipulated Voluntary Agreements:
If a respondent agrees to all or part of the plan, the Case Manager must coordinate with the County Attorney to file a "Stipulated Agreement" to dismiss or modify the active court petition.
Court Attendance & Testimony Support:
Must physically attend scheduled Circuit Court hearings or status conferences when requested by the court, the assigned MDH Regional Coordinator, or the participant's public defender.
Ongoing Progress Reviews:
Actively participate in court-convened progress conferences, providing formal documentation of the participant's treatment adherence or compliance issues to the judiciary.
Core Responsibilities Clinical Assessments:
Complete comprehensive biopsychosocial reviews for referred respondents within thirty (30) calendar days of assignment.
Outcome Monitoring:
Administer the Adult Needs and Strengths Assessment (ANSA) on established schedules.
Referral Management:
Connect participants with community-based providers within ten (10) business days of treatment plan approval.
Active Engagement:
Conduct intensive field-based outreach and home visits to ensure treatment plan compliance.
Compliance Data Entry:
Enter required participant progress notes and clinical data into the state's electronic platform within five (5) business days.
Crisis Escalation:
Report critical events (hospitalizations, arrests, housing crises) to the MDH Regional Coordinator within one (1) business day.
Minimum Qualifications Education/Experience:
Bachelor's degree in Social Work, Psychology, Counseling, or a related field plus a minimum of two (2) years of direct experience serving individuals with serious and persistent mental illness (SPMI). OR a high school diploma/equivalent plus a minimum of five (5) years of direct experience in SPMI care or mental health peer support.
Licensure & Travel:
Valid driver's license, clean driving record, and reliable personal vehicle for multi-county regional field travel.
Training:
Must complete all MDH and Treatment Advocacy Center (TAC) online onboarding training modules before independently carrying a caseload.
