WHAT WILL YOU DO?
We are seeking a Director of Quality Improvement to add to our professional team. In this position the successful candidate will be responsible for leading our agencies quality improvement initiatives, assuring compliance with state regulatory requirements, and leading Human Rights, Behavioral Plan Review, and Quality Assurance Committees. In this position the candidate will identify trends for improvement and work collaboratively with departments to develop strategies for improvement. Ideal candidate must possess excellent verbal and written communication skills, be willing to work collaboratively with other teams both internally and externally.
RESPONSIBILITIES
To direct and lead the continuous quality improvement initiatives for Community Crossroads in collaboration with Service Coordination Departments. To provide oversight and coordination of the activities of the Board Quality Assurance Committee. To monitor regional compliance with state standards including quality assurance and compliance with requirements concerning medication administration reporting and work collaboratively with other departments to address trends. To track Certification Status of service providers. In collaboration with Service Coordination Departments assure completion of Corrective Action Plans for any Service Coordination deficiencies. Complete New & Annual renewals of certification reviews for services provided through Hem-521. To represent Community Crossroads at statewide meetings and activities. Work collaboratively with Department Directors in conducting ongoing service file audits and the Director of HR for yearly Governance audit. Track agency follow through and compliance with any audit related deficiencies and assure Corrective Action Plans are submitted to the Bureau of Developmental Services as required. Oversee the tracking and report to the Board of Directors Quality Assurance committee and Bureau of Developmental Services, quality data, including but not limited to, medication errors, human rights complaints, serious incident reports, use of physical restraint, and sentinel events. Assist HR review, approval, and submission of Waivers when required for any newly hired employee including Family Managed Employees. To provide coordination, support, and leadership to the Board of Directors Human Rights Committee and Behavioral Review subcommittee. To assure agency compliance related to Rights Protection including coordination of the complaint reporting and investigation process. To track and monitor service provider and agency compliance with complaint requirements and corrective action plans.
QUALIFICATIONS A
bachelor's degree in health, human services or related field; three years' experience in the provision of services to individuals with developmental disabilities and/or acquired brain disorders. A comprehensive understanding of the principles of continuous quality improvement. Great time management skills and ability to prioritize tasks. Proficient computer skills including with Microsoft Office. Experience with HRST a plus. Interpersonal skills that contribute to collaborative and cooperative relationships with agency employees, vendor agency personnel, families, individuals served, and community members. Strong verbal and written communication skills, ability to analyze information and data and prepare comprehensive reports. Initiative and ability to work with minimal supervision.
