Job Description

Quality Improvement Coordinator Report To:
Senior Director of Quality Improvement Position Summary The Quality Improvement Coordinator is responsible for overseeing HART United's quality assurance and quality improvement activities to ensure compliance with all applicable federal, state, and local regulations. This position conducts internal audits, coordinates preparation for external audits and licensing reviews, monitors organizational performance, develops corrective action plans, and ensures compliance with requirements established by the Department of Developmental Services (DDS), Department of Social Services (DSS), Office of Policy and Management (OPM), Social Security Administration (SSI/SSA), Medicaid, and other applicable state and federal regulatory agencies. The Quality Improvement Coordinator works collaboratively with leadership, managers, and direct support staff to promote continuous quality improvement, enhance service delivery, mitigate organizational risk, and maintain compliance with all regulatory standards governing HART United's programs and operations. Essential Duties and Responsibilities Quality Assurance and Compliance Develop, implement, and maintain the organization's Quality Improvement and Compliance Program. Monitor agency operations to ensure compliance with DDS, DSS, OPM, SSI/SSA, Medicaid, HIPAA, labor regulations, and all applicable state and federal requirements. Conduct routine and targeted internal audits of programs, services, records, financial documentation, personnel files, residential programs, and operational processes. Review policies, procedures, and practices to identify compliance risks and recommend corrective actions. Monitor and track regulatory changes and communicate updates to leadership and staff. Ensure all required documentation, records, and reporting requirements are complete, accurate, and maintained in accordance with regulatory standards. Audit Coordination Serve as the primary coordinator for all internal and external audits, reviews, surveys, and inspections. Prepare the organization for state and federal audits, licensing reviews, certification visits, and compliance inspections. Organize and maintain audit-ready documentation and evidence of compliance. Coordinate responses to audit findings, deficiencies, and corrective action requirements. Monitor implementation and completion of corrective action plans resulting from audits and reviews. Maintain records of audit outcomes, corrective actions, and continuous improvement initiatives. Quality Improvement Activities Collect, analyze, and report quality and performance data to identify trends, risks, and opportunities for improvement. Develop quality indicators, performance metrics, and outcome measurements for agency programs. Conduct root cause analyses and performance improvement projects. Prepare quality assurance reports and present findings to executive leadership and governing bodies as required. Facilitate continuous quality improvement initiatives that enhance service quality, client outcomes, safety, and organizational effectiveness. Training and Technical Assistance Provide guidance and technical assistance to managers and staff regarding regulatory requirements and agency policies. Assist with the development and delivery of compliance and quality improvement training programs. Educate staff on audit preparedness, documentation standards, and best practices. Support departments in maintaining compliance and implementing corrective actions. Risk Management Identify potential compliance risks and recommend mitigation strategies. Participate in incident review processes and investigations when appropriate. Monitor agency adherence to health, safety, confidentiality, and risk management standards. Assist with maintaining accreditation, certification, and licensing requirements. Administrative Responsibilities Prepare and submit required reports to regulatory agencies and funding sources. Maintain databases, tracking systems, and quality improvement records. Participate in committees, leadership meetings, and quality improvement teams. Perform other related duties as assigned. Required Qualifications Bachelor's degree in Human Services, Public Administration, Healthcare Administration, Social Work, Business Administration, Quality Management, or a related field. Minimum of three (3) years of experience in quality assurance, compliance, auditing, program evaluation, healthcare, human services, or a related field. Knowledge of quality improvement methodologies and compliance monitoring practices. Valid driver's license and reliable transportation. Experience working with DDS-funded services, behavioral health, developmental disability services, residential programs, or nonprofit human service organizations. Professional certifications related to quality assurance, compliance, risk management, or healthcare quality improvement preferred. Knowledge, Skills, and Abilities Thorough knowledge of DDS, DSS, OPM, SSI/SSA, Medicaid, HIPAA, and other applicable regulatory requirements. Strong auditing, investigative, and analytical skills. Ability to interpret laws, regulations, standards, and policies. Excellent organizational and project management skills. Strong written and verbal communication abilities. Ability to develop reports and present findings to leadership. Proficiency in Microsoft Office Suite and data management systems. Ability to work independently and manage multiple priorities. Strong attention to detail and commitment to accuracy. Physical Requirements Ability to travel between HART United locations as needed. Ability to sit, stand, walk, bend, and operate office equipment. Ability to review records and documents for extended periods. Ability to lift up to 25 pounds occasionally. Office and community-based work environment. Occasional travel to program locations, training sites, and regulatory meetings.
