Community Relations Specialist Department:
Family Support Minimum Requirements:
Associates plus 5 years expereience in human relations field OR a Bachelor's degree in human services related field. Knowledge of community geographical awareness and of community and agency resources. Skill in customer service techniques. Skill in consultation and networking.

Week Job Duties:
Responsible for working with families as access FSP covered services. Providing assessment, case management for linkage to community mental health services, alternative community services, and residential treatment when it is determined to be medically Available to help families with obtaining, completing and submitting the FSP applications. Assisting parents in gathering information to complete the application process for the FSP program and answering questions about the program. Responsible for attending all team assisted meetings and group sessions as scheduled. Responsible for completion of and meeting deadlines for all clinical paperwork for cases, including FSP application packets, IM CANS, Psychiatric Referrals and all necessary documentation/billing notes. Responsible for collaborating with the Pathways to Success Program for youth who are dually enrolled in FSP and Pathways. Responsible for educating the community regarding the FSP Program. Responsible for facilitating positive networking relationships with community service providers and/or networks as assigned by supervisor. Individuals must demonstrate great cultural and socioeconomic sensitivity to populations served
