Job Description

Help shape Grant Gardens' new Family & Community Liaison role—connecting our program with community partners and helping adolescents and families build stronger support networks. At Grant Gardens Residential Treatment Center, our Family & Community Liaison connects our program with the families, agencies, providers, and community partners who support the adolescents we serve. Reporting to the Program Director, you'll develop referral partnerships, represent Grant Gardens through community outreach, support family engagement, and identify resources that help residents maintain healthy connections. You'll collaborate with admissions, clinical, and residential staff to coordinate outreach, address practical barriers, and support introductions to approved community services. This role focuses on relationship building, outreach, and resource coordination. Your communication, initiative, and follow-through will help families stay connected and community partners understand how Grant Gardens can support the youth they serve. Duties Build and maintain professional relationships with child welfare agencies, schools, courts, behavioral health providers, referral sources, and community organizations. Conduct outreach visits and follow-up calls, respond to program inquiries, and connect partners with admissions or program leadership. Represent Grant Gardens at approved community meetings, provider fairs, presentations, and outreach events. Coordinate facility tours and informational meetings, sharing accurate and current information about available services. Develop an outreach plan with the Program Director, maintain partner contact records, and track opportunities for appropriate referrals. Gather partner feedback and communicate service concerns, resource gaps, and outreach trends to leadership. Collaborate with the treatment team and authorized agencies to identify appropriate relatives, mentors, and other supportive adults for residents. Support approved family contact, visits, meetings, and engagement opportunities that strengthen healthy relationships. Help identify and address practical barriers to family participation, including scheduling, transportation, and access to resources. Develop and maintain a network of community providers, mentoring programs, educational supports, and family services. Coordinate approved introductions and warm handoffs to community resources in collaboration with the treatment team. Document outreach, identified resources, partner responses, and follow-up needs in designated systems. Participate in assigned treatment-team and transition meetings to share updates about family engagement and community connections. Protect confidential information, follow resident-specific contact permissions, maintain professional boundaries, and promptly report safety concerns. Experience Bachelor's degree in Social Work, Psychology, Human Services, Communications, Marketing, Business, Child Development, or a related field required. At least one year of experience in community relations, family services, behavioral health, child welfare, residential treatment, referral development, or a related setting preferred. Experience working with adolescents and families and developing external partnerships strongly preferred. Strong communication, presentation, relationship-building, customer-service, and organizational skills. Knowledge of adolescent development, trauma-informed care, family systems, and community resources. Ability to manage multiple priorities, maintain accurate records, and follow through on outstanding needs. Sound judgment regarding confidentiality, professional boundaries, resident safety, and when to seek clinical or supervisory guidance. Proficiency with Microsoft 365 and willingness to learn Welligent and other designated systems. Ability to work flexible hours when needed for family contact, presentations, partner meetings, or program activities. Must obtain and maintain CPR, First Aid, AED, Therapeutic Crisis Intervention certification, and required background clearances and training. Valid driver's license, acceptable driving record, and required automobile insurance when driving or transporting residents is assigned. Looking for a career with purpose—not just a paycheck?
