Job Description

This is a full-time position that will provide supportive preventative services to individuals who are at risk of psychiatric hospitalization to allow them to reside in the community, facilitate successful transitions between psychiatric hospitals and community, and ensure that these individuals are surrounded by community resources that will strengthen their recovery and resilience. A bachelor's degree in human services field is required and one year of paid working experience is preferred. Applicants must have a valid drivers license and transportation.
