IDD Service Coordinator

Job Description

JOB SUMMARY
The IDD Service Coordinator assist in coordinating, linking and referring eligible individuals, children and adults who have developmental disabilities to needed medical, educational, vocational, social and other identified services. The IDD Service Coordinator provides monitoring, assessment, crisis prevention and service coordination and planning to individuals in the General Revenue population, Community First Choice (CFC), Home Community Based Services (HCS) and Texas Home Living Waiver (TxHmL) Programs. Service Coordination is generally a community based service and will require contact with individuals and/or their LAR's in their home as well as in settings and natural environments utilized in the community. Monitoring frequency will occur according to assessment tool and program rules and requirements and/or at a minimum of at least every 90 days (quarterly) with the individual and/or their LAR and will include regular contacts with providers and other professional disciplines. This position requires travel in Anderson and Cherokee Counties and working a flexible schedule based on the individual needs of consumers served. This position will be responsible for executing the Authority principles of all programs as defined by the Health and Human Services Commission (HHSC).
KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge and direct experience working with individuals with intellectual/developmental disabilities; Knowledge of the department of aging and disability services (DADS) standards for IDD programs. Knowledge and application of the principles of person directed planning, individual personal outcomes and permanency planning. Ability to execute assessments of consumer needs and to develop plans that focus on individual personal outcomes. Ability to work independently and to plan and organize work effectively and manage time effortlessly to complete work. Ability to effectively communicate verbally and in writing.
REQUIRED EDUCATION AND EXPERIENCE
Education:
Bachelor's Degree from accredited university; one (1) year experience working with intellectual/developmental disabilities priority population. Bachelor's degree in a human service related field preferred
Experience:
Previous experience in IDD service coordination preferred.
REQUIRED LICENSES/CERTIFICATIONS
• A valid Texas Driver License and good driving record is required along with a copy of a valid auto insurance card
Job Type:

$18.50 - $19.02 per hour
