In this vital role, you will provide comprehensive support to clients in their homes, ensuring their safety, comfort, and well-being. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health by taking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Support medication administration following care plans and HIPAA regulations to ensure privacy and safety. Facilitate patient transfers using proper techniques such as Hoyer lifts or patient transfer methods to ensure safety during mobility assistance. Provide companionship and emotional support, engaging clients in meaningful activities to promote mental well-being. Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with home health standards. Skills Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as nursing homes or assisted living facilities. Strong knowledge of patient monitoring, Alzheimer's care, dementia care, memory care, and hospice care practices. Skilled in manual handling techniques including heavy lifting and patient transfer methods like Hoyer lifts. Familiarity with care plans, medication administration, first aid, CPR certification, and HIPAA compliance. Excellent communication skills for client interaction and team collaboration; ability to document accurately and follow detailed instructions. Experience working with individuals with autism or developmental disabilities; supporting intellectual disabilities is a plus. Ability to perform light cleaning tasks related to maintaining a safe and hygienic environment; proficient in meal preparation and personal transportation assistance.
