Job Description:
The Personal Care Aide/Transporter provides compassionate, safe, and reliable support to residents both within the community and during transportation to medical appointments and other approved destinations. This position plays an important role in supporting the nursing and care team by assisting residents with personal care needs, responding to call lights, assisting with dining and mobility, and providing safe transportation and appointment accompaniment. The ideal candidate is dependable, compassionate, safety-conscious, and able to communicate effectively with residents, families, healthcare providers, and members of the care team. Essential Functions Transportation & Appointment Support Safely transport and accompany residents to and from medical appointments, treatments, therapy, and other approved destinations. Assist residents with entering and exiting the vehicle and safely secure wheelchairs, walkers, and other mobility equipment. Remain with residents during appointments when required and provide appropriate assistance and support. Communicate appointment changes, delays, cancellations, and other relevant information to Case Management and/or nursing staff. Observe and report any changes in a resident's physical, emotional, or behavioral condition. Complete required transportation and appointment documentation accurately and timely. Maintain required transportation logs, mileage records, and other documentation. Complete required vehicle safety checks and report maintenance or safety concerns. Maintain the cleanliness and readiness of the transportation vehicle and equipment in accordance with facility procedures. Resident Care & Support Assist Personal Care staff with resident needs within the scope of the position and the resident's care plan. Respond promptly to resident call lights and requests for assistance. Assist residents with personal care, toileting, dressing, grooming, ambulation, transfers, and other activities of daily living as permitted by training and job responsibilities. Assist residents with safe mobility throughout the community. Escort residents to meals, activities, therapy, appointments, and other scheduled destinations. Assist with dining room service and resident meal needs as directed. Observe residents and promptly report changes in condition, behavior, mood, or functional ability to the appropriate member of the care team. Communication & Documentation Report resident, family, visitor, or staff concerns to Case Management and/or the appropriate supervisor for follow-up. Communicate professionally and respectfully with residents, families, medical offices, healthcare providers, and coworkers. Maintain resident privacy and confidentiality at all times. Accurately complete required documentation and records. Immediately report accidents, injuries, incidents, refusals, or unusual occurrences in accordance with facility policy. Safety & Teamwork Follow all facility policies and procedures related to resident safety, infection prevention and control, emergency preparedness, and transportation. Use appropriate safe-transfer and resident-handling techniques. Follow proper hand hygiene and PPE requirements. Maintain a clean, safe, and comfortable environment for residents. Work cooperatively with Nursing, Case Management, Activities, Dining Services, Rehabilitation, and other departments. Assist with resident-related duties throughout the community when transportation responsibilities are not scheduled. Participate in required staff meetings, orientation, in-services, and training programs. Perform other duties as assigned.

Starting at $20.00/hour
Requirements:
High school diploma or equivalent preferred. Previous experience in healthcare, personal care, senior living, assisted living, or long-term care preferred. Personal Care Aide certification preferred/required based on facility requirements. Valid driver's license and satisfactory driving record required for positions operating a facility vehicle. Ability to safely assist residents with mobility, transfers, and transportation. Strong communication and interpersonal skills. Ability to work effectively as part of a team. Dependable, compassionate, patient, and service-oriented. Ability to maintain confidentiality and handle resident information appropriately. Must successfully complete required facility orientation and training.
